Your first visit
- Fill in the online form or call 450 583-6202.
- Arrive 10 minutes early to complete your medical history.
- Bring your insurance card, RAMQ card (children under 10) or CDCP card, and a list of your medications.
- First visit: complete exam, X-rays, cleaning if time allows, and a clearly explained plan.

- Dental insurance card
- RAMQ card (children under 10)
- CDCP member card, if applicable
- List of your medications
- Recent X-rays from another clinic, if you have them

Are you accepting new patients?
Yes, we welcome new patients of all ages, adults and children. Fill in the appointment form or call 450 583-6202.
How do I book an appointment?
Fill in the online form in 30 seconds or call 450 583-6202. Reception calls you back to set a time that suits you, usually within one business day.
Do you see emergencies?
Yes. Call as soon as we open: we keep time for emergencies and do everything we can to see you the same day.
Do you bill insurance directly?
Yes, we send your claim to most insurers so you only pay the uncovered portion.
Do you accept the Canadian Dental Care Plan (CDCP)?
Yes. Bring your Sun Life member card to your visit.
